Home Health Registered Nurse (RN) - Highly Skilled and Compassionate

Job Summary:
The registered nurse plans, organizes, and directs home care services with a focus on community health education and experience. As a professional nurse, you will utilize the resources of the community to develop and implement individualized care plans for patients and their families.
Duties & Responsibilities:
Patient Care:
- Develop and initiate the plan of care in partnership with the patient, representative, and caregiver.
- Provide preventative and rehabilitative nursing procedures as ordered by the physician.
- Offer patient, caregiver, and family counseling and education.
- Prepare clinical notes and communicate with the physician and other healthcare practitioners.
Communication:
- Update the primary physician regularly and communicate changes in the patient's condition.
- Coordinate care plans with community health-related persons.
Additional Duties:
- Participate in quality assessment and performance improvement programs.
- Participate in on-call duties as defined by the policy.
- Ensure necessary equipment and services are available.
- Instruct, supervise, and evaluate home health aide care.
- Supervise Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs) and/or Home Health Aides (HHAs)/Certified Nursing Assistants (CNAs) as directed.
Job Requirements:
- Graduated from an approved school of nursing and licensed in the state where practicing.
- Minimum of two years of experience, with at least one year in public health or home care nursing preferred.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Self-directed and able to work with minimal supervision.
- Excellent observation, problem-solving, and communication skills.
- Ability to organize and prioritize workload independently.
- Able to lift, position, or transfer patients and supplies.
